I have developed a maintenance spread sheet based on the schedule in my 2011 O&M. Let me know what you think you are welcome to use as needed and change also.

Ok I tried but the DNL would not accept office 2010 spread sheed saved as 2003 xls so hopefully it will show up

I've uploaded your file as an attachment (I had to allow xlsx files, hence the original problems).
