O2 Sensor help??

check engine light came on for the downstream sensor on my OBDII reader and when i went to the live data on my obdii reader the upstream sensor it gave the approx. 0.4 volts and 2.3% while the downstream sensor gave me 1.000 Volts and N/A %.. do this mean that the sensor if burnt out?? i checked the wiring it is all fine and the fuses are fine i unplugged the sensor and no change. did any body come across this before??

Which DTC did the PCM detect? ,P0137, P0138 or P0141?. When the sensor is cold, the voltage should be below 0.16volts. It should rise up close to 5.0 volts as it gets warmer.
