Can you help me out n take a survey? (school assignment)

I have to take a bunch surveys for my "Issues in Entrepreneurship" class - and was wondering if you'd be able to fill one out for me. It's an 8-page word document (see below) that you can just fill out and email back to me - can probably be done in about 10 minutes. I'll have them all printed on the school's nickel...

The survey is only intended for those who hold a full-time "career" job (no part-time, students, freelancers, etc)
